 Grievance Procedure 

If you have a grievance concerning your experience at Processus, you may obtain a Grievance Report Form from the Business Office, and submit it to the Clinical Director. If the Clinical Director is involved in the grievance, it should be submitted to the Clinic Manager.

If after completing the grievance process you still have a serious complaint, you may contact the State of Minnesota Department of Human Services (651-296-3971).
